TOGETHER AT HOME
Together at Home proudly supports families who are either reunifying after foster care or working with social services to prevent entry into care. The goal of this program is to remove practical barriers so children can remain safely with their families.
Eligibility & Requirements:

All families requesting support must be actively working with a case manager, investigator, or prevention specialist.

Reunifying families: Eligible once the court has approved a trial home visit, with support available within 10 days of the scheduled visit and during the first year of reunification (from the date on court paperwork).

At-risk families: Working with a social worker or prevention team, even without a formal safety plan, may receive help when resources are the deciding factor in keeping children safely at home.

Family cases must be active and within the first year of involvement.

A case manager must complete and submit the form; birth families cannot apply directly.

Each form may be used for one family only, but can include requests for multiple children.

Once ready, items must be picked up at Foster Together by the case manager.
Program Support & Boundaries:

Together at Home resource requests will assist with essential items such as clothing, shoes, and toiletries.
Car seats or beds must be requested through the Safe Seats or Safe Slumber programs and follow their specific guidelines.
